For scale: the largest published safety study of synephrine ran for 60 days and tested up to 98 mg per day. The French food safety agency ANSES recommends staying below 20 mg per day and specifically advises against combining synephrine with caffeine.
The Optiloss formula contains two caffeine sources by design, green tea extract at 500 mg and green coffee bean extract at 300 mg, sitting alongside that synephrine.
This is not a reason to panic and it is not an accusation. It is a number printed on the product's own label, and the reason I put it in an article about dosing is that "two capsules a day" sounds gentle in a way that "400 mg of synephrine" does not. Both describe the same act.
Before your first capsule
Three checks, two minutes.
Count what is inside. The bottle states 60 capsules. Open it and count. A mismatch between what is printed and what is inside tells you something about the supply chain before you have taken a single dose.
Read the ingredient table once. I have gone through it ingredient by ingredient. The synephrine line is the one that matters most, for the reasons above.

"With meals" is the instruction. The most literal reading is one capsule with two of your main meals, spread across the day rather than both at once.
I would not stretch this into a rigid clock-based ritual. No published guidance specifies minutes before or after eating, and manufacturing a precision the product does not claim would be dishonest of me.
What I would pay attention to instead is consistency. Take them at roughly the same points in your day, so that if something changes, whether that is your sleep, your resting heart rate or how hungry you feel at four in the afternoon, you can trace it to a pattern rather than guess.
If you miss a dose
Nothing in the product's material addresses this, which is normal for supplements rather than medicines. The sensible approach, and the one that avoids doubling your stimulant exposure, is to skip the missed capsule and resume at the next point.
Do not take two at once to catch up. Given that the daily dose already sits four times above the highest studied amount of synephrine, doubling it is not a small decision.
How long before you would notice anything
Stimulant effects, if you experience any, tend to show up within the first few days: appetite changes, a different resting heart rate, changes in sleep.

There is no clinical trial of Optiloss itself, so there is no published timeline to hold the product to. Treat the first one to two weeks as an observation period rather than a results period.
When to stop
Stop taking the capsules and speak to a doctor if you notice palpitations, a racing heart at rest, chest discomfort, unusual anxiety or a marked rise in blood pressure.
These are the signals the safety literature on synephrine treats seriously, and none of them is something to push through in the name of finishing a bottle you already paid for.
